Why Do Sore Spots Form Under a Denture?

Most denture sores come from excess pressure at a single point. What causes them, what you can do at home, and what needs a dentist.

Almost all sore spots under a denture come from excess pressure at one point. A bump on the fitting surface only a fraction of a millimetre high, combined with hundreds of chewing cycles a day, is enough to abrade the mucosa and create a sharply outlined ulcer. The fix is usually simple: the dentist finds that point and relieves it. Once the pressure is gone the sore typically heals in three to seven days.

The usual causes

  • A border that is too long, especially near muscle attachments.
  • A small projection or sharp edge on the fitting surface.
  • An unbalanced bite that concentrates force on one side.
  • A denture that no longer fits because the ridge has resorbed over the years.
  • A denture that was broken and repaired with household glue.
  • Sharp bony projections under the mucosa.
  • Dry mouth, which increases friction.
  • Fungal infection under a poorly cleaned denture.

What the location tells you

A long thin ulcer that follows the denture border is almost always a border length problem. A single round sore at the crest of the ridge points to a projection on the fitting surface. Diffuse redness and burning is biological rather than mechanical, most often a fungal infection. Pain running under the tongue with a lower denture suggests the border extends too far into the floor of the mouth.

What to do before the appointment

  1. Wear the denture for a few hours before the visit so the contact point is visible.
  2. If pain is severe, leave it out, but bring the denture with you.
  3. Rinse with warm salt water several times a day.
  4. Avoid spicy, acidic and very hot food on that area.
  5. Clean the denture and your mouth even more carefully than usual.

What never to do

The most common mistake is filing or cutting the denture at home. It almost always targets the wrong spot, because the source of the pressure is often not where the pain is felt. Over trimming ruins the fit permanently and frequently means a new denture. Padding the denture with cotton, tissue or piles of adhesive is equally harmful: it spreads the pressure and creates new sores.

A sore that does not heal

An ulcer that has not healed within two weeks after the pressure was relieved can no longer be treated as a simple denture sore and must be examined. The same applies to lesions with hardened or raised edges, or mixed white and red areas. This does not mean something serious is present, but long lasting changes in oral soft tissue should never be postponed, particularly for people who smoke.

Lasting solutions for recurring sores

If sores keep returning in the same area, the issue is overall fit rather than one bump. In long term edentulous jaws the ridge thins and the same load is carried by an ever smaller area.

  • Relining the fitting surface and adjusting border lengths.
  • Smoothing sharp bony ridges with a minor procedure, planned as part of oral surgery.
  • Supporting the denture on two or four implants so load transfers away from the gum.
  • Reviewing abutment teeth and clasps in partial denture wearers.

Denture pain is not something to put up with; it is usually solved in one short appointment. You can reach our Muratpasa clinic for denture adjustments, and we are open around the clock for emergencies such as a fractured denture.
