Poking Wire: Immediate Relief and Definitive Fix
A wire pokes for two reasons: as the teeth align, the wire end protrudes, or the wire is displaced by impact or hard food.
Temporary relief at home
Apply orthodontic wax: soften it between your fingers, roll it into a ball and press it over the wire end. Clean cotton or sugar-free gum works temporarily.
What you must never do
Do not cut the wire with scissors, nail clippers or pliers — the fragment can be swallowed or inhaled, and an incorrectly cut wire disrupts the treatment mechanics.
Cheek and lip ulcers
Ulcers usually heal with simple measures: rinse with warm salt water several times a day. Protective gels are also available. Enlarging or febrile lesions need assessment.
Bracket and Band Problems
A bracket coming off is the most common orthodontic setback and directly affects treatment time.
What to do when a bracket comes off
If it is still on the wire, leave it in place and cover it with wax. If it came off completely, keep it and bring it — brackets are positioned tooth-specifically.
Why it should be re-bonded quickly
A lost bracket stops movement on that tooth. A short delay is harmless, but after a few weeks the tooth drifts back, losing achieved movement and extending the total treatment time.
If the same bracket keeps failing
Repeated failure at the same site usually reflects the bite: the opposing tooth may contact that bracket directly. A temporary bite raise or a different bracket type may be needed.
Emergencies for clear aligner patients
With aligners the problems differ: a cracked or lost aligner, or a lost attachment. If cracked, revert to the previous aligner — never wear a broken one. Lost attachments must be re-bonded promptly.
Preventing Orthodontic Emergencies
Most of these problems are preventable, and small habit changes markedly reduce setbacks.
