The First 24 Hours After an Extraction: Preventing Dry Socket
The whole of the healing depends on one clot protected on the first day.
Why the clot matters so much
An extraction leaves a bone-lined socket whose floor exposes bone and fine nerve endings to the mouth. The clot that forms covers this surface as both a mechanical dressing and a biological scaffold for new vessels and connective tissue. Lose it early and healing stalls rather than restarting: the bone stays exposed, the nerve endings are unprotected and pain rises sharply. A new clot rarely forms on its own because exposed bone does not bleed.
What to avoid in the first 24 hours
- Rinsing or spitting forcefully, which lifts the clot directly
- Drinking through a straw or smoking, creating negative pressure
- Probing the site with the tongue, a finger or a brush
- Hot food and drink; keep to lukewarm and soft on day one
- Heavy exercise or bending over, which can restart bleeding
- Alcohol, which delays healing and can increase bleeding
- Using mouthwashes early unless the dentist advised them
Recognising and treating dry socket
Pain should be strongest on day one and then fade. In dry socket the opposite happens: pain returns between days two and four, sharpens, radiates to the ear and temple and responds poorly to painkillers; the socket shows pale bone instead of a dark clot, often with a bad taste. It is not an infection, so antibiotics do not treat it. The socket is gently irrigated and a medicated dressing is placed, which usually relieves the pain quickly.

Dental Radiographs: What Each One Shows
There is no single type of dental radiograph, and one does not substitute for another. What each film shows — and cannot show — is different.
Three basic films
What each is used for:
- Periapical: shows one tooth's root and root tip in detail
- Bitewing: shows decay between the back teeth and the bone level
- Panoramic: shows both jaws in one frame, with lower detail
- CT: three-dimensional, for implant and impacted tooth planning
Why a panoramic alone is not enough
A panoramic covers a wide area and gives an overview: impacted teeth, cysts, the jaw joint, the sinus floor and general bone levels. It does not reliably show early decay between teeth, though, because the teeth overlap in the image. Bitewing films are therefore used for routine decay screening.
How often they should be taken
There is no fixed schedule; the interval is set by decay risk. In a low-risk adult a bitewing every two to three years may be enough, while in a patient with active decay that shortens to six months. A panoramic is usually taken every few years or when there is a specific question. Not taking unnecessary films is itself a marker of quality.
