Fluoride: What It Does, How Much, and Use in Children
With fluoride the decisive factor is the amount, not whether to use it.
What fluoride does to enamel
Bacteria produce acid on contact with sugar and that acid dissolves mineral from the enamel surface; saliva later restores the balance and some mineral returns. Decay appears when the balance tips towards loss. Fluoride helps in two ways: it speeds the return of mineral and makes the newly formed mineral more resistant to acid, while also suppressing bacterial acid production to a degree. The effect depends largely on regular direct contact with the tooth surface, which is why brushing twice a day with fluoride toothpaste is the simplest and most effective preventive measure.
Correct use by age
- Start brushing with fluoride paste as soon as teeth appear
- Under three, a rice-grain smear of paste
- Between three and six, a pea-sized amount
- A parent should dispense it; do not leave the tube with the child
- Brushing should be supervised until about six
- Spit out the excess afterwards; do not rinse with water
- Fluoride varnish can be applied for children at high risk
Excess intake and fluorosis
The concern about excess applies to a specific window: while the teeth are still forming in the jaw, up to about six years of age. Long-term swallowing of high amounts during that period can alter enamel formation and show later as white flecks or mottling on the permanent teeth, known as dental fluorosis, which is usually an aesthetic finding. In practice the risk comes from children swallowing paste, especially when too much is dispensed or a flavoured paste is appealing — hence the emphasis on age-appropriate amounts and supervision. Once the teeth have erupted, toothpaste does not cause fluorosis, so avoiding fluoride only raises decay risk.

What Smoking Does in the Mouth: Visible and Invisible
Smoking affects dental health far more deeply than staining. Its most insidious feature is that it suppresses the signs of gum disease while accelerating the disease itself.
Why suppressing bleeding is bad
Nicotine constricts the small vessels in the gum and bleeding decreases. Patients read this as healthy gums, while the inflammation continues and only the visible warning has gone. Gum disease in smokers is therefore noticed at a more advanced stage, and by diagnosis bone loss is often marked.
Wound healing and surgery
Smoking impairs oxygenation and micro-circulation in the tissue. In practice that means:
- A markedly higher risk of dry socket after extraction
- It is the commonest preventable cause of implant failure
- Graft take and the success of gum surgery are reduced
- Recovery after infection takes longer
How soon does stopping show a result?
Circulation in the gum tissue begins to recover within weeks of stopping, and the first thing noticed is usually bleeding 'appearing' — which is not deterioration but suppressed inflammation becoming visible. Every smoke-free day around surgery and during healing improves the outcome measurably.
