Digital Planning and the Mock-up in Smile Design
The aim is to trial an irreversible decision while it is still reversible.
What digital planning solves
Planning begins by assessing the teeth in relation to the face rather than alone. Photographs from standard angles and a short video of the patient speaking are taken, and references such as the facial midline, smile line, lip support and the amount of tooth visible at rest are established, then combined with a model from an impression or intraoral scan. The real gain is communication: whether the patient's mental image matches the dentist's plan is compared visually rather than verbally. Words like natural or more prominent mean different things to different people, while tooth length, width ratio and gingival level on screen are concrete.
How the mock-up is applied
The approved design is built up in three dimensions on the model, in wax or digitally, and a clear silicone key is made from it. Temporary material is placed in the key and applied in the mouth without any preparation of the patient's own teeth, so within minutes the planned smile can be seen in the mirror. This is not only about looking: the patient speaks, smiles and takes photographs, and in some cases the mock-up is left in place for a few days, testing how certain sounds are formed, how the lip closes over the teeth and whether biting is comfortable. Afterwards the material is removed entirely. If approved, preparation is guided by the thickness of that trial, which prevents removing more tissue than necessary.
Questions the try-in answers
- Do tooth length and width ratio match the expectation?
- Is the midline aligned with the face?
- Is the amount of gum shown on smiling acceptable?
- How much tooth shows at rest?
- Are certain sounds formed comfortably in speech?
- How do lip support and profile change?
- How much preparation does the planned result require?

What Curettage and Root Planing Are
A scale and polish and deep cleaning are different procedures. The first works on visible surfaces, the second beneath the gum inside the pocket, and it serves an entirely different purpose.
What is inside the pocket
In the space created when the gum detaches, calculus and a bacterial layer adhere firmly to the root surface. That region is deeper than a brush can reach and is low in oxygen, so the bacteria there are more aggressive. Until that layer is removed the gum cannot reattach to the root.
How the procedure is done
The area is anaesthetised and no pain is felt. The root surface is cleaned and smoothed with ultrasonic and hand instruments — plaque adheres less readily to a smooth surface. The mouth is usually divided into sections and completed over several visits, which is preferred for both comfort and effectiveness. Tenderness for a few days afterwards is expected.
What to expect afterwards
Several changes are usual during healing:
- Cold sensitivity, which can last weeks as the root surface is exposed
- Some recession, seen because the swelling has subsided
- Spaces between the teeth becoming more visible
- A temporary sense of looseness, then firming up
- Bleeding reducing markedly within two to three weeks
