A lost filling is often painless but leaves the tooth exposed. Protecting the cavity and booking a prompt appointment makes the repair far simpler.
When a filling comes out, the priority is protecting the cavity. The dentine underneath has lost the enamel that used to shield it, so sensitivity starts and bacteria travel inwards quickly. Even without pain, the cavity can grow noticeably within a few weeks and turn a simple refill into something that needs root canal treatment.
Why fillings fall out
It is rarely about poor material. Every bite loads the filling with hundreds of newtons of force, and hot and cold foods make the filling and the tooth expand at different rates, so microscopic gaps open at the margins over the years.
- Secondary decay under the filling undermines its support.
- Night-time clenching and grinding overload the restoration.
- Cracking hard shells, chewing ice or pens causes sudden failure.
- Very large fillings sit on thin remaining walls and last less long.
- Sticky foods can pull out temporary fillings in particular.
What to do straight away
Rinse with warm salty water to clear food from the cavity, and keep brushing the area gently rather than avoiding it. Chew on the other side and stay away from very hot or cold drinks. A pharmacy temporary filling kit can cover the cavity for a few days until your appointment. Do not pack chewing gum, wax or cotton into the hole, and there is no need to save the old filling since it cannot be refitted.
When it counts as urgent
- Spontaneous throbbing pain that gets worse at night
- Pain triggered by heat that lingers after the stimulus is gone
- Swelling of the gum or cheek, a bad taste, or fever
- Part of the tooth wall broken away, leaving a sharp edge
- Severe pain when biting on that tooth
With any of these, the problem is no longer just the filling, and a lost filling emergency assessment should check the state of the pulp.
Treatment
The dentist cleans the cavity and looks for decay underneath, which an X-ray often reveals when the eye cannot. If enough tooth structure remains, a new dental filling is placed. Where the loss is extensive, an inlay, onlay or crown gives better long-term strength. If decay has reached the nerve, root canal treatment comes first and the restoration follows.
If it keeps happening
Repeated losses usually point to an unbalanced bite or night clenching. Checking the bite and, where appropriate, using a night guard extends the life of the next restoration. Replacing the filling without addressing the cause simply resets the clock.
