Why Does Decay Start at the Crown Margin?

Why Does Decay Start at the Crown Margin?

A crown cannot decay, but the tooth underneath can. What causes marginal decay, how to spot it early and how to prevent it.

A crown itself cannot decay, because zirconia, ceramic and metal alloys are not dissolved by bacteria. Decay begins where the crown ends, along the line where its margin meets natural tooth structure. Even at a microscopic scale that line is a transition, and plaque settles there. Marginal decay is one of the most common reasons crowns have to be replaced, and it is almost entirely preventable.

What starts marginal decay

  • Plaque at the crown margin and insufficient interdental cleaning.
  • Gum recession exposing the margin and the softer root surface.
  • A micro gap developing as cement dissolves over the years.
  • A tooth that already had a large filling or root canal before crowning.
  • Dry mouth, which reduces the buffering and washing effect of saliva.
  • Frequent sugar and acid intake spread through the day.
  • Night time clenching, which fatigues the margin.

Why it is hard to notice

Marginal decay progresses quietly. The crown hides any colour change underneath, and if the tooth has already had root canal treatment there is no pain to warn you. Decay starting on a root surface also advances faster than decay in enamel, because cementum is much softer. For crowned teeth, regular radiographic review is far more reliable than waiting for symptoms.

Early warning signs

  • A ridge or step you can feel with your tongue at the crown margin.
  • Floss shredding consistently in the same spot.
  • A darkening line along the margin.
  • Bleeding limited to that area of gum.
  • New sensitivity to cold.
  • A persistent bad taste or odour.

How to prevent it

Prevention comes down to cleaning the margin line mechanically every day. Brushing alone is not enough; you need a method that reaches between the teeth. For crowned teeth an interdental brush is often more effective than floss, because it adapts better to the curved surface near the margin.

  1. Brush twice a day with fluoride toothpaste, angled towards the gum margin.
  2. Clean the margin once a day with an interdental brush or floss.
  3. Hold the brush at forty five degrees and use small vibrating strokes.
  4. Tell your dentist if you have dry mouth, as preventive measures change.
  5. Consider a night guard if you clench or grind.
  6. Have a check up every six months with bitewing radiographs when indicated.

If decay has already started

Treatment depends on how far it has gone. A very early lesion just outside the margin can sometimes be repaired with a filling without removing the crown. If decay has spread underneath, the crown is removed, the tooth cleaned and a new crown made. If it has reached the pulp, root canal treatment comes first. When too little tooth structure remains, extraction and replacement have to be discussed. That sequence shows exactly why early detection is worth so much.

Making crowns last

Having a crown does not mean the tooth needs less care; it needs more. When gum health is stable the margin stays covered and plaque accumulation drops, so regular professional cleaning directly extends the life of a restoration. The same applies to zirconia crowns: however durable the material, the natural tooth underneath still has to be protected.

If you notice roughness, sensitivity or bleeding around a crown, arrange an examination rather than waiting. Our Muratpasa clinic sees review appointments during the day and is open around the clock if a crown comes off unexpectedly.
