A second opinion is a reasonable step before extensive dental treatment. When it makes sense, and what to take with you.
A second opinion makes sense when the proposed treatment is irreversible, covers several teeth, or does not seem to match your actual complaint. It is not a vote of no confidence: dentistry often offers more than one defensible answer to the same findings, and seeing the difference makes your decision firmer. A single filling does not need one; an extensive plan reasonably does.
When it is worth doing
- Several extractions are proposed
- A full-mouth crown or prosthetic plan has been drawn up
- You cannot decide between an implant and a bridge
- Removal of a root-treated tooth is being suggested
- Pain continues even though treatment is finished
- Two dentists have given different diagnoses
- The plan is long and runs through many stages
What these share is that the decision is hard to reverse. An extracted tooth does not come back and a prepared tooth surface cannot be restored to its original state.
What to take with you
- Recent X-rays, and a digital copy of any CT scan
- The written treatment plan you were given
- A list of previous treatment and when it was done
- Your medication and medical conditions
- A note of when the symptoms started and what changes them
Asking for digital copies of your own images is entirely reasonable and avoids unnecessary repeat imaging.
Questions to ask
- Can this tooth be saved, and under what conditions?
- What alternatives exist and how do they differ?
- What would be expected within six months if nothing is done?
- What are the known risks and possible complications?
- How long will it take and how is the result maintained?

Root-treated teeth
If a root-treated tooth still hurts, extraction is not the only route. Retreatment, surgery at the root tip, or a review of the diagnosis may all be options, and sometimes the pain comes from a neighbouring tooth or the jaw muscles. See our page on root canal treatment.
Does asking cause offence?
No. Second opinions are routine, and getting one does not commit you to changing dentist. If both assessments agree, continuing with the first dentist is usually the practical choice, since your records and any started work are already there. If they differ, ask where the difference comes from.
