Sedation in Dentistry: Who Is It For and How Is It Given?

Sedation calms rather than puts you to sleep. When it is considered, which forms exist and what to expect before and after.

Sedation reduces anxiety without switching off consciousness. It is not general anaesthesia: you usually respond to questions, breathe on your own and recover quickly. It also does not remove pain by itself, so local anaesthesia is still given. Its job is to manage anxiety and reflexes, not sensation.

Who it is considered for

  • People whose fear keeps them from attending or from staying in the chair
  • Long surgical procedures
  • A gag reflex strong enough to prevent treatment
  • Patients who find it hard to stay still, including some with special needs
  • Young children whose treatment cannot be postponed
  • Cases where local anaesthesia alone is not comfortable enough

The forms it takes

The lightest is a nitrous oxide and oxygen mixture through a nasal mask, which starts and wears off quickly. Oral sedation takes longer to begin and varies between individuals. Intravenous sedation is the most controllable, since depth can be adjusted during the procedure, but it requires an anaesthetist and proper monitoring.

What is asked beforehand

Every medication you take, heart, lung, liver and kidney conditions, sleep apnoea, pregnancy and any previous problem with anaesthesia. Fasting instructions depend on the method chosen, and for intravenous sedation an adult must accompany you home.

Surgery and children

Long procedures such as impacted tooth removal make sedation worth discussing, and in cases needing oral surgery the decision weighs both duration and anxiety. With children, behaviour guidance always comes first, and in paediatric dentistry the aim is not only finishing today's work but avoiding a bad memory of the next visit.

Afterwards

After light sedation a short rest is usually enough. After deeper sedation, avoid driving that day, postpone important decisions and do not stay alone. Mild dizziness or gaps in memory of the procedure are normal; call the clinic if you feel nauseated, unusually drowsy or short of breath.

What is monitored during sedation

Heart rate, blood oxygen saturation, blood pressure and breathing are followed throughout, and monitoring becomes more detailed as the depth increases. Oxygen, suction and emergency medication have to be immediately available. Your response to spoken instructions is itself a measure of depth. Sedation is therefore a process to be observed from start to finish, not simply a dose that is given.

When it is not the right choice

  • General health conditions that make the chosen method unsuitable
  • Medication that could interact with the drugs used
  • Fasting instructions that were not followed, which usually means rescheduling
  • Airway related conditions needing further assessment beforehand
  • Short procedures where local anaesthesia alone is entirely comfortable
