When a single tooth darkens while the rest stay normal, the cause is usually inside the tooth. Here are the reasons and the options.
When one tooth darkens while its neighbours look normal, the cause is almost always inside that tooth. The usual reasons are an old injury, loss of vitality in the pulp, or pigment left behind after root canal treatment. Surface cleaning and whitening toothpaste will not change it, because the colour is internal.
Common causes
- A past impact, even one from many years ago
- Loss of pulp vitality, discolouring the tooth from within
- Residual pigmented material left in the canal after treatment
- An old, darkened filling shadowing the tooth
- Deep decay staining the structure from inside
- Structural loss after a fracture, exposing darker tissue underneath
Why a history of trauma matters
Front teeth are frequently knocked in childhood. Right after the impact the tooth may look normal and feel fine, but if the nerve and blood supply were damaged the process continues silently. A tooth turning grey years later is the visible sign of that history, so mention any past injury at your examination even if it seemed trivial.
What the examination covers
Vitality of the tooth, the surrounding tissues, and a radiograph showing the root tip and any previous root filling. Gum swelling, fracture lines, the margins of old fillings and the bite are all assessed. Skipping this and going straight to a cosmetic fix is a mistake: covering an infected root with a crown hides the problem rather than solving it. Where indicated, root canal treatment is completed first.
Treatment options
If the tooth is structurally sound and the root filling is satisfactory, an internal whitening technique may be attempted under supervision. If the colour does not lighten enough or the tooth is weakened, the surface is covered instead: a laminate veneer when most of the structure remains, a zirconia crown when the tooth needs full coverage and support, and composite bonding for limited shade differences.
No pain does not mean no problem. Once the pulp loses vitality, pain disappears too, which is why these situations progress quietly. If you notice a single tooth changing colour, have it examined even without discomfort. Our clinic in Muratpasa, Antalya is open twenty four hours a day, seven days a week.
